Appointment Information

To make an appointment, please call (704) 403-3676 and select option # 1 of the voice prompts during normal business hours. For urgent situations, we do our best to schedule a visit for the same day.

First Visit
Each new patient is asked to complete a health history form before their first visit to Carolina Internal Medicine - Concord. This will help our medical staff make a prompt and accurate assessment during the first visit. Please bring your current insurance cards, including Medicare or Medicaid cards, and any current prescription bottles. Please arrive 10 minutes before your scheduled appointment to complete registration and other paperwork, or print and complete the online forms before your arrival.

Routine and Sick Visits
The goal of Carolina Internal Medicine - Concord is to provide patients with prompt, easy access to quality healthcare. Our medical staff is available to see patients by appointment during normal office hours. Please arrive 10 minutes before your scheduled appointment.

Always bring your insurance cards, including Medicare or Medicaid cards and all prescription medications.

After Hours Care
In the event of a true emergency, please call 911. If an non life-threatening emergency arises after hours or on holidays, please call our main office line at (704) 403-3676 and follow the prompts for after hours emergency care.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
